How it works

Three easy steps to get started

Step 1

Call on their behalf

Tell us who you are helping. We ask a few short questions about their coverage — no insurance card needed to start.

Step 2

We handle the details

With their authorization, we work with their insurance and healthcare provider to gather the prescription and documentation their plan needs.

Step 3

Free delivery to their door

Once approved, their CGM supplies are shipped directly to their home at no shipping cost.

Why families consider a CGM

More visibility. Less guesswork.

  • See your glucose levels throughout the day, not just at the moment you test
  • Reduce reliance on traditional fingersticks where appropriate
  • Notice trends and patterns — how meals, activity, and sleep affect you
  • Share readings with a caregiver or your healthcare provider
  • Make day-to-day diabetes management easier to understand

Common questions

Answers before you call

Can I call for a parent or family member?
Yes. Tell us you are calling for someone you care for. We will explain what we need from them and from their provider, and we can speak with you directly once they have authorized it.
Will Aptiva contact my doctor?
Only with your permission. If you would like us to, we reach out to your provider's office for the prescription and the supporting documentation your plan needs, then keep you updated on where things stand.
Does Medicare cover continuous glucose monitors?
Medicare covers CGMs for some people with diabetes when specific medical criteria are met and the device is prescribed by a treating provider. In general, coverage requires that you manage your diabetes with insulin or have a documented history of problematic low blood sugar. Whether you qualify depends on your diagnosis, your treatment, and your particular plan. Our team can verify your benefits and tell you what your plan requires.
Do I need a prescription?
Yes. A continuous glucose monitor requires a valid prescription from your healthcare provider, along with documentation supporting the medical need. With your authorization, Aptiva can contact your provider's office to request it, so you are not the one making the calls.
How much will I have to pay?
We cannot tell you before we check, and we will not pretend otherwise. Your cost depends on your plan's coverage, deductible, and copay or coinsurance. We verify your benefits first and tell you the expected out-of-pocket amount before anything ships. If it is more than you want to pay, you are free to stop.
What happens when I call?
We'll ask a few short questions, review your coverage, and explain what your plan requires. There's no obligation to order anything. Call (800) 429-1720 whenever you're ready.
